Contact ER/HR to discuss appropriate title, duties, responsibilities, requirements, salary and effective date. Once these details are finalized, ER/HR will need the following information to prepare the waiver for review and approval.

  1. Name of Employee
  2. Proposed Title
  3. Proposed Department
  4. Proposed Salary
  5. Effective Date
  6. FOAP
  7. Justification
  8. AVSL Approver

Attach the following documents:

  1. Updated resume
  2. Current and Proposed Org Charts
  3. Job Description
