Below are forms used for Human Resource processes, procedures, and transactions for University Administration employees on the Urbana-Champaign, Chicago, and Springfield campuses. For UA units serviced by Employee Relations and Human Resources, all completed forms should be submitted to ER/HR for review, approval, and/or processing. UA units not serviced by ER/HR should submit completed forms to their unit's Human Resource contact.
