Below are forms used for Human Resource processes, procedures, and transactions for employees of the system offices at the Urbana-Champaign, Chicago, and Springfield locations. For system offices serviced by the HR Service Center, all completed forms should be submitted to the HR Service Center for review, approval, and/or processing. System offices not serviced by the HR Service Center should submit completed forms to their unit's Human Resource contact.
