Accidental Death & Dismemberment (AD&D) Insurance

Accidental death and dismemberment (AD&D) insurance is a voluntary benefit intended to supplement your life insurance coverage. AD&D coverage can help to offset costs or lost wages due to certain serious irreversible injuries.

State AD&D coverage is available to most State benefits-eligible employees.

University AD&D coverage is available to University employees working a regular appointment of 50% or more.

Eligibility

To be eligible for State AD&D insurance, you must:

  • Be eligible to participate in the State Universities Retirement System (SURS), and
  • Have a regular appointment of at least 50%

There is no coverage available for dependents.

Cost

The cost of the State AD&D plan is $0.02 (two cents) per month per $1,000 of coverage. You pay the full cost for your coverage through payroll deduction.

The State AD&D plan is administered by the Minnesota Life Insurance Company.

Coverage begins on the date you elect coverage or your State benefits eligibility date, whichever is later. You may enroll at any time as long as you are eligible for coverage.

You may elect coverage in the following amounts:

  • An amount equal to basic State life insurance coverage only, or
  • An amount equal to the combined basic and optional State life insurance amounts, not exceeding the lesser of either 5 times basic life insurance coverage or $3,000,000.

Insurance benefits are paid depending on the cause and severity of injury, and other conditions. Please see the AD&D section of the Minnesota Life Certificate of Insurance for details.

During a seasonal layoff, illness, family/medical or educational leave without pay, coverage will continue as long as you continue to pay your portion of the premium, which will be billed directly to you.

During other types of leave without pay, coverage may continue for up to 24 months, but you will be responsible for both your employee premium and the State cost for coverage. Contact University Payroll and Benefits for more information.

University AD&D Insurance

Eligibility

To be eligible for University AD&D insurance, you must be a permanent and continuously employed faculty/staff member with an appointment of at least 50%.

All retirees under age 70 at retirement who are immediate annuitants under SURS and were insured for 12 months prior to retirement are also eligible.

Coverage for spouse and children is available in the “Employee Plus Family” package.

University AD&D coverage is available at the rates listed below. You pay the full cost for your coverage through payroll deduction.

The University AD&D plan is administered by The Hartford.

Coverage begins the first day of the second month following the date your enrollment is received. For example: If your enrollment is received during the month of January, the effective date of coverage will be March 1.

You may elect coverage for the following amounts:

  • For yourself, one of the amounts between $25,000 and $300,000 listed in the cost table above.
  • When you elect the family package, the coverage is:
    • For your spouse, 40% of your own coverage, or 50% if no children.
    • For a child, 10% of your own coverage, or 15% if no spouse.

Dependent children covered under this plan must be unmarried and 18 years of age or younger (up to age 22 if a full-time student and financially dependent upon you for support).

Your enrollment in the University AD&D plan also includes travel assistance services through Europ Assistance USA. Travel assistance is available when traveling, nationally or internationally, 100 miles or more from your primary home.
